Innoviant has made every effort to ensure that our ID cards meet current NCPDP standards. The image below is a sample of the Innoviant ID Card. Below that is an explanation of required information needed to successfully submit a claim to Innoviant

Required Information for Claims Submission

To ensure that there is little or no disruption in the processing of claims, the fields listed below will be required in the submission of any claim for payment.
Information from the front of the ID card
  1. RxBin number 610127
  2. RxPCN number 02330000
  3. RxGrp number (varies for each member, see below)
  4. Cardholder ID
  5. Cardholder name
  6. Person Code
Additional Required information not on the ID Card:
  • Date of Birth (DOB)
  • Usual & Customary (U&C)
  • Correct Days Supply for quantity dispensed
  • Physician DEA
