Group Specific Information provided below
Required Information for Claims Submission
To ensure that there is little or no disruption in the processing of claims,
the fields listed below will be required in the submission of any claim for
payment.
Information from the ID Card:
- RxBin number
- RxPCN number
- RxGrp number (varies for each member, appears on ID Card)
- Cardholder ID
- Cardholder Name
Additional Required information not on the ID Card:
- Person Code (See note below)
- Date of Birth (DOB)
- Usual & Customary (U&C)
- Correct Days Supply for quantity dispensed
- Physician DEA
Note: Unfortunately the person code is not on the card.
The Pharmacist will have to ask for it.
Cardholder will be Person Code (00).
