Innoviant Mail Order Program
The Innoviant Mail Order Program allows prescription products to be ordered through the mail service
pharmacy. If your plan sponsor has elected this program, prescription maintenance products, used to
treat chronic conditions such as, but not limited to, arthritis, diabetes or high blood pressure, can
safely be prescribed for a period of 90-days. Using mail order offers Innoviant members the advantage
of obtaining up to a 90-day supply of prescription products. Members can obtain a mail order enrollment
packet by calling the number on the back of your ID card or by contacting
Innoviant Sales & Marketing.
Innoviant members with a mail order benefit on their group prescription program can access the mail
service pharmacy to fill or refill their eligible mail order prescriptions online. The member will be
charged based on the participating mail order pharmacy co-pay amount listed in their summary plan description.
If your company is enrolled in the Innoviant Specialty Pharmacy Program, please note that specialty
medications should not be submitted to a mail order pharmacy. To order specialty medications, please
use the
Specialty Pharmacy Program.

pharmacy-Innoviant Rx. This change is effective on Jan. 1, 2005. Innoviant Rx is fully integrated with
Innoviant, giving you access to a customer service staff that services both retail and mail
order in one environment.
